  3. Tuberculosis (TB) Medical Form + Syphilis Medical Form
    Download Physical Exam Form (PDF)
  4. $75 Payment Cash or check made payable to: State of Hawaii

  2. Application for New Tattoo Artist License
  3. Tuberculosis and Syphilis Report Form
